Job Description:

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and barriers to care, recommending personalized care plans
Coordinate care between members, families, and providers to ensure access to services
Provide ongoing monitoring and education to members and their families on healthcare options

Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
2 - 4 years of related experience in care management
Registered Nurse (RN) licensure in the state of Arizona or Compact State Licensure
Experience with social determinants of health and community resources
Ability to document and maintain compliance with regulatory standards
